1. Photo ID number found on the order form and the information sheet enclosed with your original photograph.

2. Your complete name.

3. Your home mailing address.

4. Your telephone number.

5. Your email address.

6. A brief description of the problem.

Our policy is that if possible we will correct any problems within ten (10) business days from the date of notification.  We apologize for any inconvenience.
